Sinopsis:

La historia del juego tiene lugar en el sistema de metro de un Moscú post-apocalíptico, donde jugaremos desde la perspectiva del personaje llamado Artyom, quién nació antes de que tuviese lugar la guerra y la ciudad aún existía.

El juego se localiza en una serie de túneles reales del metro de Moscú, en los que existe una desesperada lucha contra unos seres desconocidos y donde el jugador deberá aventurarse en una misión en la que se jugará la vida. Deberá recorrer los distintos niveles del metro enfrentándose a un fenómeno desconocido que afecta a los seres humanos provocándoles un terrible dolor de cabeza que llega a matarlos, un ataque al que Artyom aunque no es inmune sí es capaz de resistir.
Después de producirse el holocausto nuclear en la superficie no queda ningún humano. Las zonas subterráneas, como el metro, se han convertido en el hogar de la humanidad. En estos refugios las personas se resguardan y tratan de sobrevivir, pero ya no basta con eso, tendrán que defender su hogar y enfrentarse a los seres desconocidos en cada rincón del metro, pase lo que pase hasta el último aliento.
